The Federal Aviation Administration is now investigating the incident, KOVR reported.

The sheriff’s office said that the woman’s primary parachute and the reserve one tangled together, The Sacramento Bee reported.

KCRA reported the chute was tangled around the woman, who was identified as Sabrina Call, 57, of Watsonville, California.

The owner of Skydive Lodi Parachute Center, Bill Dause, said Call had done nearly 2,000 jumps over her lifetime and had started diving from the facility in the past year.

“No one actually saw exactly what happened until probably the last couple hundred feet,” Dause said, according to the newspaper. “She had two parachutes out, entangled with each other that weren’t fully inflated, and pointed toward the ground at very rapid speed.”

Call did not, however, pack her own parachute before the accident, an employee of the company said, saying that it was parked by a contract parachute rigger, according to The Sacramento Bee. The employee spoke after Dause told media outlets that he didn’t know who packed Call’s chute.

KOVR reported that the business has had more than 20 deaths since opening in the 1980s.

The company was ordered to pay $40 million after a deadly jump five years ago that killed an 18-year-old man after his parachute didn’t open. Officials found that the man’s instructor was not properly certified, KCRA and other media outlets reported.
