MATTHEWS, N.C. — Federal investigators said serious pilot errors and possible fatigue caused a UPS cargo plane to crash last year in Birmingham, Alabama.

The report said the pilots incorrectly programmed the plane's flight management computer, descended too fast, failed to call out altitude levels and didn't abort the landing when they realized they weren't lined up properly.
One of the pilots who died on the plane was Captain Cerea Beal, Jr., from Matthews. He spent 23 years with UPS.
Earlier this year, a report revealed that Beal had complained about work schedules that didn't allow enough time for sleep.
