Salisbury nursing home being investigated after reports of several violations

SALISBURY, N.C. — The North Carolina health department is investigating a skilled nursing home in Salisbury after reports of several violations.

Accordius Health on Statesville Boulevard is trusted to take care of 185 elderly and vulnerable residents. A 51-page document lays out the state’s investigation.

The investigation tells the story of 10 different residents, including a man with a diagnosis of dementia and psychosis. He had a documented history of eating items from the trash, and was seen by the director of nursing possibly eating a mouse.

The state found the staff needed to be reeducated about being better about notifying doctors when incidents happen.

The report also documented a series of fights and attacks by a resident with Alzheimer’s. It said the resident hit one resident across the face, scratched another and pushed a third.

The investigation also found COVID-19 violations, including that the facility failed to report a resident who tested positive and failed to test staff members for the virus.

Channel 9 reached out to Accordius Health but have not heard back at this point.
