Police said the children were last seen with their mother Joreka Izquierdo. She is believed to be experiencing mental health issues.

A family member contacted police Friday around 6:37 p.m. in concern for the mother and children.

Police believe Izquierdo is heading to Miami, Florida.

If anyone has information on the whereabouts of Izquierdo and her five children, they are asked to call 911.
